Welcome to the Canine Health Survey.   In order to participate in the survey, you must be a registered member.   Registered members may login and continue their data entry or view reports or update information on any dog they have already entered in to the database.   The registration is for security purposes and only requires an email address (for lost or forgotten passwords), a username of your chosing, and a password.

There is NO connection or association between your registration and the information that you will submit in the canine health survey that follows.
